Other Important Information - 2010

We want you to be informed about your health care. Please click on the following links for more important information.

Quality Improvement Program

Passport Advantage has developed a quality improvement program to make sure that you get the right medicines to safely treat your medical conditions. We also want to make sure that we have network pharmacies in enough locations so that you can easily fill your prescriptions.

Each year, we check on how well we and our network providers do these things. To decide if we are doing well, we:

  • Look at information about the drugs our members take.
  • Look at what the most recent medical research says about using prescription drugs.
  • Ask network providers and plan members to help us decide how we may need to improve our services.

Then, we work with our network providers and pharmacies to make these changes.

Medication Therapy Management Program

Passport Advantage offers a special program for members who:

  • Have chronic medical conditions
  • Use many different drugs
  • Use high cost drugs.

This program is called a Medication Therapy Management Program. If we think that we could help you improve your health through this program, one of our pharmacists will contact you. You do not have to take part in this program. If you want to opt out of this program, call Member Services.

Our Contract with Medicare

Passport Advantage contracts with Medicare to give our members health and prescription drug coverage. Passport Advantage or Medicare could end or change this contract. Also, the Plan could change its service area. If this happens, we will let you know. Then, you can get your Medicare benefits through Original Medicare and a Medicare Prescription Drug Plan, or through a different Medicare Advantage plan.

Please Report All Suspected Fraud and Abuse

Tell us if you suspect fraud or misuse of Passport Health Plan or Passport Advantage ID cards or benefits by a member or provider. To report fraud or misuse, please call Passport Health Plan’s Fraud and Abuse Hotline at 1-866-833-9718.

If you have identified billing practices that cause you to suspect potential fraud or abuse, you may call the OIG's National Hotline at 1-800-HHS-TIPS (1-800-447-8477) to report the activity. Suspicions of fraud or abuse may also be reported to Medicare's Customer Service Center at 1-800-MEDICARE (1-800-633-4227) or by TTY at 1-877-486-2048.

Benefits, formulary, pharmacy, network, premium, and/or co-payments/co-insurance may change on January 1, 2011. Please contact Passport Advantage for details. Premiums, co-pays, coinsurance, and deductibles may vary based on the level of help you receive. Please contact Passport Advantage for further details. The monthly plan premium is $0 for Passport Advantage members, for both medical and prescription drug benefits. This is generally lower because members receive Extra Help from Medicare. If you are a Passport Advantage member, that means you are receiving full Medicaid benefits and Kentucky Medicaid is paying your monthly Medicare Part A and Part B premium. If you lose your eligibility for full Medicaid benefits, you may have to pay for these premiums. For full information on Passport Advantage benefits, call our Members Services at 1-800-578-0603, then press 1, (TTY: 1-800-648-6056) 7 days of the week from 8 a.m. to 8 p.m.
