Passport Advantage (HMO) has “plan providers” who have agreed to provide you with your health care services. You may go to any of our plan providers listed in the following directory. Referrals are not required.  Find a provider now! 

If you are a Passport Advantage member, you can choose from over 1,000 pharmacies in our network. This is even more than the Medicare program requires for us to have in our network. In most cases, your prescriptions are covered under Passport Advantage only if they are filled at a network pharmacy. Find a pharmacy now! 

What if you use non-plan providers or pharmacies to get covered services?

Remember: If you have an emergency, you do not need to use a Passport Advantage provider or pharmacy. Get care right away!  Read our emergency policy. 

This directory is for our service area, which includes the following 16 Kentucky counties: Breckinridge, Bullitt, Carroll, Grayson, Hardin, Henry, Jefferson, Larue, Marion, Meade, Nelson, Oldham, Shelby, Spencer, Trimble, or Washington. We also list pharmacies that are in our network but are outside of our service area.

You may go to doctors, specialists, and hospitals in or out of network. You may have to pay more for the services you receive outside the network, and you may have to follow special rules prior to getting services in and/or out of network. Eligible beneficiaries must use network pharmacies to access their prescription drug benefit, except under non-routine circumstances when they cannot reasonably use network pharmacies. Passport Advantage has retail, long term care, and home infusion pharmacies available in our network. In general, Part D benefits are only available at contracted network pharmacies.
