Case Management Program

Case management is a program for Passport Advantage members who have one or more health care needs or disabilities. If you can answer yes to any of the following questions, the case management program may help you!

Do you have many medical problems?
Do you need help managing your health care?
Do you have more than one health care provider?

Who Will I Talk To?

When you call, you can work with a case manager. Case managers are registered nurses or medical social workers. They can talk with you about your health care needs. They can also work with your providers to help you get the care you need.

Passport Advantage is a health plan with a Medicare contract. This Plan is available to all anyone who is an active Passport Health Plan members and who has Medicare Parts A and B. Eligible beneficiaries may enroll in the plan at any time. Please call Passport Advantage for more information. Benefits, formulary, pharmacy, network, premium, and/or co-payments may change on January 1, 2012. Copays may vary based on the level of Extra Help you receive. Contact Passport Advantage for more details. Eligible beneficiaries must use network pharmacies to access their prescription drug benefit, except under non-routine circumstances.
