    Effective September 1, 2011, Passport Health Plan (PHP) and Passport Advantage (PAD) will reimburse pharmacies for administration of the injectable seasonal flu vaccine to certain members. Eligible PHP and PAD members have a $0 copay for the flu vaccine.

    To assist you with identifying medications that should be avoided in the elderly, the Plan is pleased to present our new High Risk Medication Guide for Adults Age 65 and Older. This guide is based on the Healthcare Effectiveness Data and Information Set (HEDIS) list of high risk medications and the Beers Criteria for Potentially Inappropriate Medication Use in Older Adults.

    Effective March 1, 2011, Passport Health Plan (PHP) will mirror this state requirement and the diabetic supplies listed below will no longer be available through Durable Medical Equipment (DME) providers. These supplies will only be covered through the pharmacy with a valid prescription.
